Admission Process

  1. Pick up the 2009-2010 Enrollment Application or the 2010-2011 Enrollment Application from the front reception area of any campus, or access and print the application online.
  2. Complete the entire application and turn it into the front reception area of the enrolling campus. Be sure that all of the pages in the application have been completed.
  3. Bring all of the following copies with you:
    1. Letter- Typed or handwritten letter stating why you would like your child to attend Life School
    2. Copy of State Certified birth certificate (Hospital copy is not acceptable)
    3. Copy of Social Security Card
    4. Copy of updated immunization record/or certified exemption of immunization form
    5. Copy of all assessment reports as follows:
      1. Grades 1st - 3rd
        1. Previous year’s TPRI student report or equivalent
        2. Most recent report card. If applying for the next school year, a complete report card from the current school year must be provided as soon as it becomes available
      2. Grades 4th - 8th
        1. Previous year’s TAKS student report or equivalent
        2. Most recent report card. If applying for the next school year, a complete report card from the current school year must be provided as soon as it becomes available.
        3. Last year’s full year report card
    6. Grades 9th-12th
      1. Official Transcript
  4. Copies of any special program information from previous school. This includes special education, 504, GT, and Speech.
  5. Copies of disciplinary records if no conduct or citizenship grade is listed on the report card. If there are no discipline records, then there will need to be a letter or other documentation from the school so indicating
  6. Copy of Home Language Survey from previous school if English is not the primary language.
